iPad Faculty Request Form

This form is specifically designed for individuals seeking to borrow an iPad for extended durations, surpassing a single semester. Please note that if you require an iPad for a shorter period, such as a few weeks or a single semester, we kindly request that you reach out to our helpdesk for assistance with a short-term loan.

By signing below, all parties assume the following:

• The iPad, which is the property of the Brearley School, will be used exclusively for academic purposes.
• The {academicDepartment} department assumes full responsibility for the care and safekeeping of the iPad.
• The iPad will not be modified, altered, or have any unauthorized software or applications installed during the loan period without the knowledge of the Technology Department.
• Any damage or loss of the iPad must be reported immediately to the Director of Technology.
• The iPad will be returned, with all accessories included, on or before the agreed-upon end date in the same condition as it was borrowed.  All data collected by the faculty member will be wiped and not saved upon return.
